Certified Nurse Assistant (CNA) Responsibilities:
- Provide hands-on personal care and patient assistance including, but not limited to the following activities of daily living: Bathing, oral hygiene, grooming, dressing/undressing, toileting, meal prep/feeding, and home management tasks.
- Act as an extension of nursing and therapy services as ordered and directed within the Certified Nursing Assistant scope of practice such as assisting with range of motion exercises and monitoring and documenting vital signs.
- Assist with medication reminders and administration per Certified Nursing Assistant guidelines.
- Assist with transferring and ambulation as needed.
- Observe and report any changes or concerns regarding the patient’s condition.
- Evaluate, address, and appropriately escalate patient and caregiver concerns to ensure patient and caregiver satisfaction.
- Ensure the safety of the patient is considered at all times
- Document patient visits utilizing Homecare Homebase (HCHB), AHN Healthcare@Home’s EMR, through a company-provided tablet.
Hospice Certified Nursing Assistant Qualifications:
- Must have and maintain in good standing a Certified Nursing Assistant (CNA) certification and state required licensure.
- One-year prior experience as a Certified Nursing Assistant preferred.
- Valid driver’s license with reliable transportation.
- Possess and maintain CPR certification.
- High standard of professionalism, confidentiality, ethics, and compliance.
- Ability to document within an electronic medical record.
