Home Health
Home Health nurses keep patients at home and out of the hospital. They empower their patients to take ownership of their health and recovery through care coordination, education, and prevention.

Hospice Care
Hospice nurses manage patient pain and symptoms and help them to plan for end-of-life care. They also educate patients on end-of-life processes and direct and comfort family members on spiritual support options and other available hospice therapies.

In This Role, You Will:
- Deliver safe, accurate, patient-centered care in collaboration with interdisciplinary teams.
- Clearly educate and empower patients and families to build confidence in care plans and support positive outcomes.
- Use your strengths to solve challenges while ensuring thorough and compliant documentation.
Qualifications:
- Active RN license in the state in which you work with at least one year of clinical experience delivering quality care.
- Valid driver’s license and ability to travel to patient locations (~20% of the day).
- Strong time management, tech proficiency (EMR, mobile devices), and commitment to timely charting.
