We are hiring a Nurse Practitioner for a Hospice position providing end-of-life care to patients in their homes in the New Kensington, PA area. This position is eligible for a $ 10,000first year success bonus to reward the accomplishment of key milestones during your first year!
What You’ll Do:
- Provide compassionate, patient-centered hospice care, collaborating with an interdisciplinary team to enhance comfort, dignity, and quality of life for patients and families
- Conduct comprehensive clinical assessments, manage complex symptoms and medications, and develop individualized plans of care that align with patient goals and end-of-life preferences
- Educate and support patients, families, and caregivers through clinical decision-making and care planning while ensuring accurate, timely, and compliant EMR documentation through strong critical thinking and problem-solving skills
- Work independently, but never alone
- Travel to patient homes within assigned territory

Nurse Practitioner Qualifications
- Bachelor of Science (BSN) degree from an accredited School of Nursing
- Master of Science (MSN) degree from an accredited School of Nursing
- A minimum of five (5) years of clinical experience
- Hold, in good standing, a license as a Registered Nurse and Nurse Practitioner issued by the state in which you work
- Experience in a community based Home Health or Hospice/Palliative Care practice is strongly preferred
