Home Health
Home Health nurses keep patients at home and out of the hospital. They empower their patients to take ownership of their health and recovery through care coordination, education, and prevention.
Hospice Care
Hospice nurses manage patient pain and symptoms and help them to plan for end-of-life care. They also educate patients on end-of-life processes and direct and comfort family members on spiritual support options and other available hospice therapies.

Hospice Registered Nurse Responsibilities:
- Assess the physical, psychological, social, and spiritual needs of the patient and caregivers and formulate a comprehensive Interdisciplinary plan of care.
- Coordinate with physicians, caregivers, volunteers, ancillary therapies, and home health aides to support the care and needs of the patient.
- Educate on prescribed medications and supplies and ensure optimal quantities.
- Oversee and document home health aide plan of care every two weeks.
- Attend interdisciplinary team meetings to evaluate the plan of care and revise as needed
- Provide support at time of death and period of bereavement as needed.
- Document patient and family care plans, actions, goals, and concerns in Homecare Homebase (HCHB), AHN Healthcare@Home’s EMR, through a company-provided tablet.
Hospice Registered Nurse Qualifications:
- Must hold, in good standing, a license as a Registered Nurse, issued by the state in which you work.
- A minimum of one (1) years of experience working as a Registered Nurse including some medical/surgical experience and/or an acute care or home care setting is preferred.
- Must understand the hospice philosophy and team approach to hospice care
- Must be able to transfer patients as needed according to proper techniques of body mechanics to ensure patient and staff safety.
- Must have strong communication and organizational skills and high-standard of ethics and compliance.
- Ability to document within an electronic medical record.
- Associate Degree from an accredited School of Nursing or higher preferred.

The low end of the range includes compensation related to salary and first year success bonus. The high end of the range is inclusive of productivity bonuses and additional shift work that is paid at a premium (i.e., after hours, weekends, on-call). *Compensation potential varies by market.
