Home Health

Home Health nurses keep patients at home and out of the hospital. They empower their patients to take ownership of their health and recovery through care coordination, education, and prevention.

Hospice Care

Hospice nurses manage patient pain and symptoms and help them to plan for end-of-life care. They also educate patients on end-of-life processes and direct and comfort family members on spiritual support options and other available hospice therapies.

Homecare Homebase (HCHB) experience is preferred for this position.

Compensation: $33.50 - $40.50

The base compensation range for this role is fixed, with a maximum cap of $40.50. We want to be transparent about this as we continue discussions.

Hours: Saturday & Sunday, 8AM - 5PM EST or CST

Provides overall triage assessment, case management and coordination via telephonic and telehealth care to patients.   Assures optimal coordination of patient care across the continuum for all disease management and triage patients as assigned. Provides telephonic triage to all incoming patient/caregiver calls as assigned.  Works in conjunction with the Clinical Supervisors, Customer Service staff, field staff, and on-call staff to coordinate care to best meet the patients’ needs efficiently, assuring outstanding customer service and meeting the highest possible standards of quality of care.

Home Health Telehealth Registered Nurse Essential Duties and Responsibilities

  • Direct interaction, education, and follow-up with patients and caregivers as assigned by the Telehealth Administrative Assistant.
  • Triages all incoming patient/caregiver calls for assigned shifts/times.  
  • Adheres to the established policies and procedures when caring for disease management/telehealth program patients or when triaging patient calls.
  • Guides patients in chronic disease management concepts:  patient self-management, medication/diet compliance, physician follow-up, etc. 
  • Acts as a patient advocate in coordinating care within the organization and outside the organization.
  • Documents all patient/staff interactions.
  • Attends departmental and other staff meetings as assigned.
  • Contacts physicians with updates of patient status and to receive direction/orders as needed.
  • Communicates with all providers involved in patient care of the Disease Management/Triage patients to assure coordination of care.
  • Participates in IDT meetings  
  • Occurrence Reporting - follow appropriate process with patient/customer grievances/complaints and occurrence.
  • Maintains proficiency with computer systems.
  • Maintains current knowledge of pharmacology, disease management and chronic disease processes.
  • Thorough knowledge of and strict adherence to the company’s Mission, Vision, and Core Values.
  • Performs other duties as assigned by management.

Home Health Telehealth Registered Nurse Qualification Requirements

To perform this job successfully, an individual must be able to perform each essential duty satisfactorily.   • Licensed RN with current state license.
  • Excellent telephone and communication skills required.
  • 2-3 years of clinical practice in their respective field.
  • 1-3 years of home health/hospice experience.
  • Knowledge of disease management and chronic disease processes.
  • Prior management experience preferred. 
  • Ability to work independently and within a multidisciplinary team. 

Certificates, Licenses, Registrations

Must have and maintain in good standing professional license, certificate, or registration, as applicable.
